MHR!The role of the Case Manager is to provide services that will
stabilize their client's condition, provide assistance and support
to prevent hospitalization and to focus on the recovery process. In
addition, having responsibility to carry out rehabilitation and
support functions and assist in treatment, education, support, and
consultation to families, and crisis intervention in consultation
with the multi-disciplinary team. This position engages clients and
establishes trusting collaborative relationships directed toward
the goal of competitive employment in community job settings with
other workers without psychiatric disabilities. A few of the

Responsibilities:
- Coordinate integrated services and supports necessary to assist
each client as they work towards their goal of recovery.
- Provide collaboration and education to significant others and
family members who are interested in assisting the client in their
recovery process.
- Recognize significant changes in client status, symptoms,
safety risks and life circumstances in a timely manner.
- Collaborate with the Clinical Supervisor or Team Leader as
appropriate, prioritize client needs and intervene in crisis
situations in a timely and effective manner.
- Work in partnership with clients to develop an Individual
Community Support Plan that identifies services and supports that
will enable each client to live as independently in the community
as possible or to develop an Individual Family Community Support
Plan that identifies services and supports needed by the client and
their family to improve the well-being of the child and improve
family functioning. Services and supports might include activities
such as obtaining and maintaining benefits, housing, or other
services; preventing crises; monitoring mental, chemical, physical,
dental, and environmental health, enhance daily living skills and
family functioning.
- Assure that services are implemented as developed by the
client, family and team as documented in the Individual Community
Support Plan or Individual Family Community Support Plan, and that
services are continuously monitored and evaluated.
- Treat all clients with dignity and respect, providing services
that are respectful of diagnosis, age, gender, and culture.
- Assure that all changes in client status or needs are
communicated through documentation.
- Assure all mandated civil commitment procedures and processes
are followed and information is submitted according to timelines
required by law.
- Assure that clients and families are informed about the
complaint and grievance procedures.
- Act in a manner to assure client confidentiality and privacy
that complies with MHR policies and procedures/HIPAA/MN Medical
Records Act.
- Assure that MHR productivity standards (i.e., Assertive
Outreach, case load, face-to-face contact, AIMs, chart management
and data entry) are met, as outlined by the Team Leader & Clinical
Supervisor.
- Provide coverage for team members to assure and maintain
continuity of care for each client served.
- Participate in all scheduled staff meetings and trainings,
unless excused by the immediate supervisor.Minimum Education and
Experience:
- Holds a bachelor's degree in social work, psychology or human
services related field from an accredited college or university and
meets requirements for supervision and continuing education.
- At least 2000 hours or more of supervised experience in the
delivery of mental health services to children.
- Skilled in the process of identifying and assessing a wide
range of recipient needs.
- Knowledgeable about local community resources and how to use
those resources for the benefit of the recipient.
- Without a bachelor's degree meets one of the following:
- Has 3 or 4 years of experience as a case manager associate
- Be a RN without bachelor's degree with 3 or more years combined
specialized training in psychiatry and work experience consisting
of community interaction and involvement or community discharge
planning in a mental health setting.Compensation details:
